What Does the Strongest Evidence Show?
The clearest result comes from a 2024 systematic review and meta-analysis published in Dentistry Journal, pooling 10 randomised controlled trials with 825 participants aged 18–69 (Roberts et al., Dent J, 2024).
For topical propolis in recurrent aphthous stomatitis — recurring mouth ulcers, commonly called canker sores — the findings were:
- Time to complete pain resolution fell by an average of 4.18 days (95% CI −5.59 to −2.77).
- Redness around lesions dropped significantly (MD −2.95, 95% CI −3.21 to −2.69).
- Participants were markedly more likely to show reduced lesions at three months (RR 5.58, 95% CI 1.88–16.51).
Two things deserve equal billing. Healing time itself — as distinct from pain — showed a smaller effect whose confidence interval crossed zero (MD −1.92, 95% CI −5.36 to 1.52), meaning that result wasn't statistically reliable. And the reviewers graded certainty across all outcomes as very low, citing risk of bias, heterogeneity, and small samples.
Gum Health and Oral Inflammation
Beyond ulcers, propolis mouthwashes have shown promising results for plaque and gingival inflammation in systematic reviews of gingivitis and periodontitis management. Propolis now appears routinely in toothpaste, mouthwash, and oral sprays for this reason.
The mechanism is consistent with the hive's own use: antibacterial activity against oral bacteria, combined with anti-inflammatory effects from the flavonoid fraction.
This is an adjunct, not a replacement. Nothing in the evidence suggests propolis substitutes for brushing, flossing, or a dentist.
What About Immune Support?

A useful label states the propolis type or botanical origin, the extraction solvent, and a marker compound content such as total flavonoids or galangin. Many list only "propolis extract" and a milligram figure, which conveys almost nothing about what's in the bottle.
