There is no standard propolis dose, and the reason is structural: propolis composition depends on which plants the bees visited, so a gram from Brazil and a gram from Poland are different substances. Trials use concentrations from 1% to 20% and never agree on a milligram figure. The one contraindication that matters is allergy — propolis is a top-20 allergen in North American patch testing, which is rare company for a supplement.

Why Propolis Has No Standard Dose
Because propolis isn't a compound. It's whatever resin the bees could reach, mixed with wax and enzymes, and its active chemistry changes with the local flora. European and North American propolis is poplar-derived and rich in flavonoids like pinocembrin and galangin. Brazilian green propolis comes from Baccharis dracunculifolia and is defined by artepillin C, which poplar propolis barely contains. Red propolis from Cuba and Brazil is different again.
So "500 mg of propolis" specifies a weight and almost nothing else. Compare that with the shilajit trials, where the tested extract was standardised to a minimum fulvic acid percentage — imperfect, but at least a number. Propolis has no equivalent in common commercial use. Any dosage guide that gives you a confident milligram figure is quietly pretending this problem doesn't exist.
What Doses Have Trials Actually Used?
For oral rinses, the range is a factor of twenty. A 2020 systematic review in BMC Oral Health pulled together nine trials of propolis mouthwash across 333 subjects and catalogued what they used: 10% propolis in ethanol, raw propolis diluted 1:1 with water, 5% propolis, 20% propolis with food-grade alcohol, and a 1% aqueous extract (BMC Oral Health 2020;20:198). Five countries, five formulations, no two alike.
The mucositis literature is tidier because the outcome is harder, but it still doesn't produce a dose you can copy. A meta-analysis of five randomised trials in 209 cancer patients found propolis mouthwash cut the incidence of severe oral mucositis substantially, with an odds ratio of 0.35 (95% CI 0.18–0.70, p = 0.003) and no heterogeneity between studies (PMID 30022350). A clear effect, from five different preparations.
What that tells you is worth holding onto: propolis appears to work across a wide range of concentrations, which is reassuring about safety margins and useless for precision dosing.
How Many Milligrams Is 20 Drops?
Nobody can tell you, and that's the practical heart of this article. Our own tincture label says 20 drops in water three times daily during illness, dropping to 10–15 drops three times daily afterwards, and 10 drops three times daily for prevention. Reasonable-sounding instructions. Now try to convert them.
A drop is roughly 0.05 mL, so 20 drops is about 1 mL and the acute schedule comes to 3 mL a day. To turn that into milligrams of propolis you need the extract's concentration — and our label doesn't state one. Neither do most.

Same bottle, same instructions, a three-fold spread in what you actually take. Until a tincture states its strength, treat the drop count as a ritual rather than a dose, start at the bottom of the label range, and judge by response rather than by arithmetic. We'd rather say that than invent a number.
The Alcohol Nobody Counts
Propolis resin doesn't dissolve in water, which is why tinctures are alcohol extractions — typically around 70% ethanol. That's a design necessity, not a shortcut, but it has a consequence that labels never mention.
At the acute schedule of 3 mL a day, a 70% tincture delivers roughly 2 mL of pure ethanol daily. For most adults that's trivial — about a sixth of a standard drink. It stops being trivial for anyone avoiding alcohol entirely: people in recovery, people on metronidazole or disulfiram, some religious observances. A propolis tincture is not an alcohol-free product and shouldn't be treated as one.
The paediatric instruction deserves a harder look. Our label suggests children aged 5 to 15 take a drop count equal to 1.5 times their age, three times daily — 12 drops for an eight-year-old, so 36 drops or about 1.8 mL of tincture a day. At 70% ethanol that's roughly 1.3 mL of alcohol daily for a child. Not an acute hazard, but not nothing either, and it's the kind of decision that belongs with a paediatrician rather than a label. If you want propolis for a child, an alcohol-free aqueous extract or a propolis lozenge sidesteps the question entirely.
Is Propolis Allergy Common?
Common enough that it's the main safety story. Propolis sits in the top 20 allergens of the North American Contact Dermatitis Group's standard series, with positivity in the region of 3% to 5% of patients tested. In 2007–2008 it reacted in 4.9% of 5,085 patients tested, making it the eleventh most frequent allergen in the series (PMID 23340394), and it rose significantly again in the 2019–2020 round of 4,121 patients (PMID 36917520).
One caveat matters enormously and is almost always dropped: patch-tested patients are people who already have suspected contact dermatitis. They are not a random sample of the public. The true population rate is lower. What the figure does establish is that propolis belongs in the same conversation as nickel and fragrance mix, which is not where most people file a bee product.
Reactions are usually contact dermatitis — itching, redness, sometimes cheilitis around the mouth from lip balms or oral rinses. Systemic reactions after ingestion are rarer but documented, and anyone with a known bee-product allergy should treat propolis as off-limits rather than as something to test cautiously.
Why the Allergy Rate Depends on Where the Propolis Came From
This is the point where the dose problem and the safety problem turn out to be the same problem.
In October 2019, North American patch-test panels switched their propolis material from Chinese to Brazilian propolis. Positivity rates jumped — and de Groot and Rustemeyer's 2013–2025 review of the literature describes the effect as catching experienced researchers by surprise (doi:10.1177/17103568251392243). Brazilian propolis is simply more sensitising than poplar-type propolis.
So the honest answer to "how allergenic is propolis?" is: depends which propolis, exactly as the honest answer to "how much is a dose?" is: depends which propolis. Botanical origin governs both. A product that tells you its source region is giving you more useful information than one that gives you a milligram figure without one.
Who Should Not Take Propolis
Anyone with a known allergy to bees, honey, royal jelly, beeswax or pollen. This is a firm no rather than a caution — the cross-reactivity is real and there's no benefit worth the risk.
People with asthma should be careful, since bee-product sensitivity can present as bronchospasm rather than a rash. Pregnancy and breastfeeding fall into the standard no-data category: no trials have enrolled these groups, so the honest position is to ask a clinician rather than to reassure. Anyone avoiding alcohol should choose a non-tincture form. And anyone already sensitised to fragrance markers is somewhat more likely to react, since cross-reactivity between propolis and fragrance allergens is documented.
How to Test Your Own Tolerance
Two steps, and they take a week between them.
First, a skin test: a drop on the inner forearm, left for 24 to 48 hours, checking for redness or itch. This catches contact sensitivity before you find out the harder way. Second, if the skin test is clear, start at a quarter of the label dose — five drops rather than twenty — for three days before moving up. Delayed reactions are the norm with propolis rather than the exception, so an immediate absence of symptoms isn't the all-clear.
Stop at any sign of mouth soreness, lip swelling, rash or wheeze. Contact cheilitis from oral propolis is easy to mistake for chapped lips, which is exactly why it gets missed.
How Long Should You Take It?
Short courses have the better support. The mucositis trials ran for the duration of cancer treatment, typically a few weeks; the gingivitis trials mostly ran two to four weeks. Nothing in the literature tests propolis taken daily for months, so continuous long-term use is untested rather than established.
Practically, that argues for using propolis the way our own label's acute schedule implies — through an illness or a defined dental problem, then stopping — rather than as a permanent daily supplement. It also reduces the cumulative sensitisation risk, which rises with repeated exposure rather than with any single dose.
