Chaga is the one functional mushroom with a documented way to seriously hurt you, and it doesn't work like the others. There's no allergic reaction, no acute toxicity, no dramatic drug interaction. There's oxalate, taken in grams, every day, for months — and three published cases of kidney failure that all follow the same arithmetic. One of those patients never got his kidney function back.
At ordinary doses chaga is well tolerated. The real risk is oxalate nephropathy from high daily intake sustained over months: published cases involved 3 to 15 grams a day for periods from three months to five years, and two of the three needed dialysis. Measured oxalate content varies five-fold between brands — 2.8 to 14.2 grams per 100 g — and no label declares it. Never combine chaga with a vitamin C supplement, and avoid it entirely with kidney disease or a stone history.
Most chaga side-effect pages open with "generally safe" and mention kidneys in passing. That gets the emphasis backwards. Chaga's risk profile is unusual enough to be worth reading properly, and it's almost entirely about dose multiplied by time.
Why Is Chaga's Risk Different?
Because it's not a reaction — it's an accumulation. Chaga is exceptionally high in oxalate, the same compound behind calcium oxalate kidney stones. Oxalate isn't metabolised; the kidneys excrete it. Feed them more than they can clear, for long enough, and crystals deposit in the tubules.
That mechanism has no early warning. There's no rash, no nausea, nothing telling you to stop. The published cases present at the point where the damage is already done, which is what makes this worth taking seriously in a way that a list of mild digestive complaints isn't.
What Do the Published Kidney Cases Show?
Three cases, and their consistency is the point. Each involved gram-scale daily intake sustained over months or years, in someone taking chaga for a chronic condition.

The 2020 case is the sobering one. A healthy-kidneyed 49-year-old took chaga for eczema, escalated the dose when it didn't work, and ended up on permanent dialysis (Lee et al., J Korean Med Sci, 2020). Escalating a supplement that isn't working is ordinary behaviour. Here it was the mechanism.
The 2022 case recovered completely after a month of dialysis and steroids, which suggests the damage is reversible if caught early enough (Kwon et al., Medicine, 2022). Three months of exposure was reversible. Five years was not.
How Much Oxalate Is Actually in Your Chaga?
You can't know, and that's the most useful finding in this literature. When the 2020 case authors measured the product their patient had used, it contained 14.2 grams of oxalate per 100 grams of powder. They then tested two other brands: 11.2 g and 2.8 g per 100 g.
That's a five-fold spread, and no chaga product on the market declares an oxalate figure. Follow it through: 3 grams of the low-oxalate powder delivers about as much oxalate as 0.6 grams of the high-oxalate one. A dose in grams tells you almost nothing about your actual exposure.
Which means the honest advice can't be a number. It has to be a behaviour: moderate amounts, real breaks, and never the vitamin C stack.
Why Is Vitamin C the Worst Thing to Combine It With?
Because ascorbate is metabolised to oxalate in the body. Taking a vitamin C supplement alongside a high-oxalate one adds a second oxalate source on top of the first, which is precisely the combination in the 2022 case: 10 to 15 grams of chaga daily plus 500 mg of vitamin C.

Are There Real Drug Interactions?
Fewer than the kidney story, and none with a case report behind them. The mechanistic concerns follow chaga's effects on glucose and platelets.

What Are the Ordinary Side Effects?
Minimal, which is part of what makes the kidney risk easy to miss. Reported effects at normal doses are digestive: mild stomach upset or loose stools, usually early. Some people find strong chaga decoctions harsh on an empty stomach.
There's no allergy signature, no dermatitis pattern, and no liver signal in the literature. Chaga is genuinely uneventful right up until it isn't, and the thing that changes is cumulative dose.

- Anyone with chronic kidney disease or reduced kidney function — this is the clearest contraindication of any mushroom in our range

How Do You Take Chaga More Safely?
- Keep the daily amount modest. The case reports start at 3 grams a day sustained over years; there's no reason a general-wellness routine should approach that.
- Take real breaks. Chaga is one of the few mushrooms where cycling has a mechanism behind it rather than folklore — cumulative oxalate load.
- Never stack it with a vitamin C supplement.
- Drink water. Dilute urine is the simplest protection against crystal formation.
- Take it with a calcium-containing meal, which binds oxalate in the gut before absorption.
- Don't escalate the dose because it isn't working. That's the exact pattern in the 2020 dialysis case.
- If you take it daily and long-term, ask for kidney function on your next blood test.
